- Description: <p>ARTT is a small multidisciplinary team who provides a city-wide service to the Sheffield Community. <br /><br />ARTT provides holistic assessments and slow stream, long term rehabilitation to patients who have a diagnosis and/or deterioration of a long term condition. Our aim is to proactively manage long term conditions.<br />Patients must be over the age of 16 and have potential for, and ability to engage in, rehabilitation. <br /><br /><strong>There are 2 streams to our service:</strong></p> <p><strong></strong></p> <ul class="bull pad-left2 mar-bot1"> <li><strong>Therapy</strong> for individuals who have Long Term Conditions (LTCs).</li> <li><strong>Falls clinic</strong> – providing a <strong>one-off assessment</strong> for patients within their outpatient clinic appointment.</li> </ul> <br />ARTT operates as a hybrid service model, combining home visits with use of an outpatient gym and therapy room based within the Nether Edge Outpatient Clinic (NEOC).<br /><br />ARTT is <strong>NOT</strong> an urgent response service.<br /><br />For ARTT therapy:<br />Referral via email: <a href="mailto:sth.arttsheffield@nhs.net">sth.arttsheffield@nhs.net</a>.
