Target's Letting Individuals Flourish Together Service (L.I.F.T)
Service, is for adults with SEND and / or a mental illness. It provides support in two ways through a core and cluster model.
Our Core offers a mixture of shared supported tenancies and self-contained flats. These are owned or managed by Target with either staff offices and facilities on site or increased staff attendance accordingly to need. Our Staff are available throughout the week and have the ability to flex support up and down in keeping with the needs of the people we work with. These homes furnished and maintained by us.
The second part of our service provides dispersed floating support to people within their own homes throughout the wider community. These homes may or may not be owned by us however people living within these have a separate tenancy agreement from their support package ensuring their home remains unaffected by their changing needs or wishes.
All our staff work in a trauma and psychology informed manner, encompassing Trauma Informed Care (TIC) and Psychology Informed Environments (PIE) and this is embedded into all our projects and teams. In addition to this, they receive training in neurodiversity, autism and learning disabilities, management of medications (although this service offers prompts and observations only), eating disorders, mental illnesses and crisis management along with working in with other key partners to ensure a holistic and robust support package tailored to suit the preferences and wishes of those in receipt of key workers sessions.
What are the aims of the service?
Staff develop positive working relationships with the people we support, in partnership with their existing support network, in order to assist them to make positive moves to improve and maintain their health, wellbeing and independence.
What are our objectives of the service?
We offer a range of practical and emotional support tailored to the individual’s preferences, choices and wishes. As such, no two days are the same as our activities are as varied as our people.
Some of the support delivered may include:
- Support with budgeting and managing finances.
- Support with meal planning and cooking.
- Meal planning and shopping.
- Help with planning and attending appointments including those relating to Health, Training and Education.
- Support to maintain their home.
- Promoting self-image and esteem by encouraging self-care.
- Support to manage medications.
- Support to engage in the community and events including accessing public transport.
- Support with correspondence, navigating utility bills and setting up payments.
- Support to plan moves into independent accommodation
Referral
Target is an approved provider on Sheffield City Council’s Independence and Support Framework, and also Sheffield City Council's Learning Disability Supported Living Framework. As such Community Mental Health professionals and Social Workers can should refer you via the appropriate Framework pathway with SCC by contacting brokerage@sheffield.gov.uk
All our service users have been assessed by a professional within Sheffield City Council as having eligible social care needs, and have an agreed care package. If you do not have a Social Worker, you will need to contact the First Contact Team on tel:01142734908 to explore how you can access their support.
