Learning Disability Service - Case Register
The Case Register is a list of people in Sheffield with Learning Disabilities. It is used to help people that plan your services to get information so that the right type of support is available when needed.
The Case Register can be used to send you details of new services and to ask you and your family what you think of existing services. You don’t have to be on the Case Register, you decide if you want to be included. It helps the people who plan services to make sure the right support is there. The information is also used for research. We won’t give them your name and address unless you agree.
The Home Visitors who work in the Case Register will come and see you periodically to update your details, and signpost you to other services where needed.
